Q. How do I change my personal information?

A. Click Personal Information section at top. Change information that is incorrect. Click Submit Changes Button at the bottom. The changes will be reflected in your Electronic Health Record.

 

Q. How do I change my password?

A. Click Patient Information at the top. Click Change Username and Password to left. Enter current password, new password and confirm new password. Click Update button.

 

Q. How do I complete a shortened version of my Medical History?

A. Click iForms at the top. Click green header named Health History. Answer questions and click Mark as Viewed button at the bottom.

 

Q. How do I update my insurance policy information?

A. Your insurance policy can be reviewed on the patient portal but changes are made in person at New York University College of Dentistry.

 

Q. How do I view my walkout statement?

A. Click Documents at the top. Choose date of service, Click Patient Approval Report to view a copy of the walkout statement.

 

Q. How do I confirm my appointment?

A. Click appointment section at the top. Choose desired date, Click green confirm button. If the appointment is already confirmed, you will see a blue header above your appointment information.

 

Q. How do I make a payment to my online account?

A. Please Click on “Online Bill Payment” link at the bottom of the page.
